uhh..I revise my answers. After thinking hard about your very specific clue for #46, I think it's You Raise Me Up by Josh Groban.
And while #40 has got to be In Your Eyes, you probably wouldn't chart Peter Gabriel in 04, so I'm going to guess it's that horrible remake by some dude/band/whatever named Mitch Allan.

I don't think anyone fits the #28 clue.
The only song by a group that peaked at #15 in 2004 that I can think of was D-12's "How Come", and that was their third to reach the Top 40.

28) OK, in 2004, Lean Back & Falls On Me peaked at #14. But, neither of those singles were the second top 40 singles for this band from the album.
