When I considered responding to this topic, my initial reaction was "man, I'm going to have to dig into the archives for this one." However, this year alone, many ballads (or "modern rock ballads") or whatever you would like to describe them, have actually hit #1 on my chart this year alone.

You know new grounds are being tread when I must ask whether particular songs on this list....are actually, in fact, ballads.

This doesn't comprise my top 10-20 ballads of all time, per se (at least judging by my point value system), but these are at least #1s since I began posting PC's at PMB:

Notice the sharp increase in ballads in recent years. That's a phenomena that I never noticed until I compiled this list. Truly amazing.

It seems that calling some of these songs “ballads” may be stretching it, but I must admit I’m not entirely clear to what constitutes a ballad, and perhaps some of the songs on my list are not really ballads, and others which I omitted because I thought they were borderline might actually be considered ballads. But here’s what I could think of now. I didn’t stick to 10 or 20 because just too many songs came to my head that I love too much. I did 80 like I do on my weekly chart. It’s very hard to put them in order since they’re very close.

I don't have a personal chart but I do have quite a good idea of my favourite 5 ballads of all time. Listing any more would be sort of difficult for me because my rankings are really static (though my top 5 usually remains pretty much the same although songs shift spots), so I'm keeping it a little shorter than what was asked for. I also restricted my selections to love ballads specifically, otherwise I would take a century for my selections.
