An overreliance on a handful of producers at the expense of standout singer/songwriters who've been stars at other formats, and especially Hot AC, since none of the most respected musicians of the past two-plus decades have been welcome at top 40 radio in ages.
The fact that none of the major record labels had the wisdom to simply purchase Napster, which have resulted in the music industry being in much better shape financially speaking today.

I'd say that most musical bigwigs do not view the signing of Taylor Swift as a mistake, considering she (along with Lady GaGa and select others) has been keeping the industry afloat the last few years. Also, at least Taylor has her considerable songwriting skills to fall back on, unlike many of her peers.
