I feel like "Circles" could have potentially at least come close to this, had it not been for the album bomb that booted it into recurrent status. I'm pretty sure it was still at #18 in its final week on the chart, and it is a song with exactly the kind of chart run to return to the top 20 from the 21-25 range. On the other hand, holiday music would have also been a very difficult obstacle for it to overcome, with the 52-week recurrent rule in effect.

Common misconception, but technically Ariana's album bomb that week didn't really knock Circles out of the top 25. I mean, yes, 34+35 (and Dakiti) debuted in the top 10 the week it got knocked out, but no other song from the Positions album bomb debuted in the top 25. I'd argue Circles was on the cusp anyway due to its radio losses, which likely stemmed from Hot AC and AC starting the switch over to Christmas music that week. It may not have gone recurrent if 34+35 and Dakiti hadn't debuted so high (or at all), but it likely would've been close.

Even if Circles would have survived that week, its run in the Top 20 was done. It may have spent 2 more weeks in the 21-25 range on the 11/21 and 11/28 charts, but for sure, there was no way Circles was ever going to survive the BTS and Christmas blitz of 12/5. Even after Christmas, Circles fell short of re-entering the Top 25 to chart again.

I imagine so. It's at #48 on the 04/17/2021 chart, and "RAPSTAR" and "Kiss Me More" alone should push it to recurrency. Add the "Fearless" album bomb and any potential re-entry from DMX, and it seems pretty much guaranteed at this point.
