Is it common for artists/A&R executives to be unhappy with how an album turns out, and then decide not to release it? Are albums permanently shelved, or started over from scratch at least?
Any examples come to mind?
I can only think of a couple: Kiley Dean and 50 Cent.

Angela Via (3 albums), Ashley Ballard, R-Angels, Dream ("Reality"), Willa Ford ("Sexysexobsessive"), Rubyblue, Shannon McNally, Paula Cole, Michael Fredo ("Weather Vane"), etc.

Arista Records released Lisa 'Left Eye' Lopes' "Supernova" everywhere else except the US to see how it would do. Unfortunately, they didn't enjoy the numbers.
"Supernova" was delayed until it was finally cancelled in the US.

Just read on another board that Fiona Apple's new album is done but Epic isn't happy with it (no singles). The album may not see the light of day on that label.

Two high profile ones are Usher and Monica-- lead single flops let them to go back in to the studio and re-do the albums, even though they were complete. I don't know what Usher's original album title was, but after "Pop Ya Colla" bombed, and he said the album got leaked, he went back and made "8701". After "All Eyez On Me" underperformed, and she said the album leaked (well it did), Monica went back and made "After the Storm". In both cases though they kept parts of the original album.

I think it was called 'All about U' ... Yep. Judging from the titles of his first three singles from "8701", he should've kept that title. :)

Kelis "Wanderland" wasn't released in the US. Though "Flashback" was kept for the Tasty album and Popular Thug (a different version) appeared on The Neptunes Present: Clones.

50 Cent's Power of the Dollar.
1999, he was signed to Columbia, but heavy bootlegging scrapped this album and left 50 out of a record deal. Well, we obviously know what happened next.

R-Angels -- Where's The Party Cheesy, crappy girl group signed to Motown around 2000-2001. Album was never released, but fully pressed and manufactured copies, complete with packaging, were available in the Columbia House record club for a time.
Tamyra Gray recorded an album for J Records, but didn't like the material given to her. She was dropped, signed with 19's new recording label, and then released The Dreamer, a completely different collection with mostly self-penned songs.

Linkin Park completed an album while they were touring for [Hybrid Theory] that could've been released in 2002 but they "scrapped" it, re-released [Hybrid Theory] in the form of remixes (ReAnimation) and proceeded to create METEORA.
Although the instrumental for "Somewhere I Belong" was saved for the heap and became used for a single.
